How to Submit a Request
To submit a legal or medical record request, please follow the steps below:
- Register at https://www.chartswap.com/register
- Sign in and search for the name of the Ambulance Service you would like to receive records from.
- Enter request details and upload all required supporting documentation
- Once your request has been reviewed and records are available, you will receive a notification and invoice
- Payment may be made directly through the platform by check, credit card, or debit card
